    In our November 28, 2025 response to the Westboro Coroner's Report Recommendations, OPTA established a new Safety and Training Committee, where recent serious transit bus collisions will be included as a standing item. This committee will provide a structured forum for members to share lessons learned, discuss contributing factors, and advance best practices using a safe systems approach. Sabrina Pasian, Chief Safety Officer for OC Transpo has volunteered to Chair the new committee. Karen Lusk, Supervisor of Transit Safety and Training for York Region Transit has volunteered to be Vice Chair.
